   Case Study: Physician Contract Expiring

Situation:

  • A large tertiary care facility's Chief of Staff relays to Medical Affairs that a Pediatric Intensivist is not performing at an expected competence level
  • The provider's contract expires shortly, and focused reviews must be done
  • Other peer subspecialists are unable to conduct peer review due to economic constraints
  • The unit nurses are concerned with the unusual and erratic therapeutic interventions and lack of responsiveness to questions

QMCG Team Role: Conduct an objective review of focused case events to assist in performance documentation to enhance review of suitability for continued employment:

  • Peer review physician matched with similar experience, tenure, patient volume and acuity
  • Reviewer addressed standard of care determinations, opportunities for improvement and suggestions for skill base improvement
  • Offers to conference in person or by phone given

Peer Review Conclusions:

  • Immediate concerns found with basic intensive case management practices

Medical Staff/Hospital Accomplishments:

  • Report to be used with ongoing performance improvement data in order to make sound judgment and employment decisions.
