   QM Services: Accreditation and Licensure Readiness
       Assessment Survey

GOALS:

To provide the client preparing for an accreditation or licensure survey with an assessment of readiness in an interactive manner which provides assessment as well as solutions to areas in need of improvement.

To obtain private accreditation of various agencies, as desired;

To build effective processes for internal maintenance and performance improvement activities.

WHEN NEEDED:

This service is particularly valuable to clients with rehabilitation, long term care or skilled nursing facilities who are anticipating an accreditation or licensure survey within a twelve month period. QMCG’s third party assessment lends objectivity to the process and provides the client with solutions to issues needing resolution before the accreditation or licensure surveys.

DESCRIPTION:

Effective guidance for participants in the Medicare and/or Medicaid programs at all stages of the survey process in order to achieve substantial compliance with the regulatory requirements, including:

Pre-survey reviews (outcomes focused):

  • Analysis of existing compliance efforts in clinical care documents and organizational materials

  • Review of systems and processes to assure compliance with governing documents

  • Review of survey preparation activities and overall readiness

  • Interviews of key individuals responsible for functions under review to assure readiness for survey

  • Process evaluation, recommendation and support

  • Written assessment and critique providing recommendations and guidance for improvement

  • Draft or revise documents as needed/desired

Concurrent survey support and guidance:

  • Can attend survey to clarify current processes and compliance efforts

  • Will review findings/comments on a daily basis

Post-survey design and implementation of plans of correction to obtain substantial compliance:

  • Survey report analysis

  • Post survey correspondence

  • Process streamlining

  • View process planning and implementation

DURATION:

This service can be completed within a 4-6 week time frame depending upon scheduling of on-site component of project. QMCG remains available throughout accreditation or licensure survey preparation to assist with implementation of suggested recommendations. QMCG will also schedule "on-call" availability on the day(s) of the survey to answer specific questions and provide guidance on issues raised during the course of the survey.
