Modifier -25

An article was recently published to the National Government Services (NGS)
website that should serve as a reminder for billing a visit code with modifier
-25.
Modifier -25 should be listed with an E & M visit code only when a
significant, separately identifiable service (that is above and beyond the
usual care associated with a procedure) is performed during the same visit for
a particular patient.
Appropriate documentation to support the E & M visit code and the procedure
must be noted to support the modifier -25 being listed with the visit code.
